  • Glaucoma
  • Hepatitis C
  • Inflammatory bowel disease
  • Multiple Sclerosis
  • Pain that is either of the following nature: (i) Chronic and severe; or (ii) Intractable
  • Parkinson’s disease
  • Positive status for HIV
  • Post-traumatic stress disorder
  • Sickle cell anemia
  • Spinal cord disease or injury
  • Tourette’s syndrome
  • Traumatic brain injury
  • Ulcerative colitis

State Agency

Physician Certification

Physicians can authorize patients for medical marijuana and ARE NOT required to have a bona fide provider-patient relationship with the patient. Doctors are however required to obtain an active certificate to recommend (CTR) from the Ohio Department of Health.
